PERSONAL DATA PROTECTION APPLICATION FORM

GENERAL INFORMATION

Pursuant to the Law No. 6698 on the Protection of Personal Data (“KVKK Law”), personal data subjects defined as the “relevant person” (“hereinafter referred to as the Applicant”) are granted certain rights to submit requests regarding the processing of their personal data under Article 11 of the KVKK Law.

In accordance with paragraph 1 of Article 13 of the KVKK Law, applications to be submitted to our Company, acting as the data controller, regarding these rights must be delivered to us in writing or through other methods determined by the Personal Data Protection Board (“Board”).

Within this framework, written applications to our Company may be submitted by printing out this form and sending it to us through one of the following methods:

  • By personal application of the Applicant (with identity verification),

  • Through a notary public,

  • By being signed with a secure electronic signature as defined under the Electronic Signature Law No. 5070 and sent to our Company’s registered electronic mail (KEP) address.

Other application methods that may be determined by the Board will be announced by our Company when applicable.

Response Period and Fees

Applications submitted to our Company will be responded to within thirty (30) days from the date they are received, depending on the nature of the request, in accordance with Article 13/2 of the KVKK Law.

If the process requires an additional cost, the fee determined by the Personal Data Protection Board may be charged. Responses will be delivered to the Applicant in writing or electronically, in accordance with the relevant provisions of the Law.

    (Choosing e-mail will allow us to respond faster.)

  • I want to receive it in person
    (If collected by proxy, a notarized power of attorney is required.)

 

This application form has been prepared to correctly identify your relationship with our Company, fully determine any personal data processed, and ensure that your request is answered accurately and within the legal time limit.

In order to prevent unlawful data sharing and ensure data security, our Company reserves the right to request additional documents (such as a copy of an ID card or driver’s license) for identity verification purposes.

Our Company shall not be held liable for applications submitted with incomplete, outdated, incorrect, or unauthorized information.
